Cochlear Implant

Hearing loss is a high incidence in society. The rate of gore birth hearing loss is 30 in 10,000 births, which is well above the incidence of other anomalies such as cleft palate, down syndrome, limb anomalies in childbirth. For this reason, in 2004, the Ministry of Health initiated a national newborn hearing screening protocol jointly with some Universities, and all newborn babies across the country were scheduled to undergo this screening. Inonu University Medical Faculty Turgut Ozal Medical Center Ear Nose Throat As a department of the Department of Child Health and Diseases and Gynecology and Obstetrics Branches, with support from the beginning of our hospital, we started a newborn hearing screening program as an individual application. In 2009, following the completion of the Newborn Hearing Screening Trainer Training organized by the Ministry of Health, our faculty member from the KBB Department of Animal Husband participated in our hospital screening program and took part in the program as an advanced diagnostic and treatment center.

In recent years, there has been hope for those with impaired hearing loss and congenital absence of cochlear implants (bionic ear), which can not gain the ability to speak well as a result.

The Cochlear Implant is an electronic device designed to help those with hearing loss at advanced and very advanced levels who can not benefit from hearing aids. The cochlear implant system converts everyday sounds into coded electrical stimuli. My stimuli stimulate the hearing nerve and the brain perceives them as sound.

The cochlear implant system consists of two parts:

  • The internal part implanted with the operation-the implant,
  • The outer part called the speech processor.

In a person with a cochlear implant, sounds are picked up by the microphone at the receiver of the implant and converted to electrical signals. These signals are transmitted to the speech processor and encoded therein. These stimuli are surgically delivered to the electrodes placed in the cochlea. The auditory cortex takes these stimuli to the brain. The brain also perceives these signals as sound.

Children with congenital hearing loss from cochlear implants may benefit from children and adults who subsequently experience hearing loss. A cochlear implant is suitable if there is a hearing loss due to an internal source and if the hearing aid can not obtain sufficient speech information. It is difficult to predict exactly how much the person can benefit from this practice. However, children who have congenital hearing loss benefit greatly if they can be implanted as young as possible. The benefit rate after 5 years is very low. For this reason, it is preferable to apply a cochlear implant before 5 years of age, preferably under 3 years of age, in cases of congenital hearing loss. With a well-tuned hearing aid, however, a cochlear implant is not necessary if the person is able to adequately work and understand it. If the main cause of hearing loss is not the inner ear or there are various inner ear anomalies, cochlear implantation may not be beneficial in these patients. In these cases, different approaches may come into the picture sometime.

After learning to speak, they benefit from cochlear implantation even if they lose their hearing for any reason at any age. However, the longer the hearing loss is, the less likely he is to benefit from a cochlear implant. For that reason, implant treatment may be more beneficial in the early period.

Given the benefits of cochlear implant implantation, Almost all users benefit from the Implant because they will reach the ability to hear more environmental sounds. Thus, it is possible to have a closer relationship with the person's environment. People will be safer because they will hear voices like traffic, alarm, siren. In addition, users will hear speech voices through a near-thigh cochlear implant. However, it should not be forgotten that the understanding of the voices of speech will take a certain time, especially in children, and it will require special education for children. For most users after implantation, it facilitates daily life. After some time it is possible to understand how to speak without lip reading, some users can also gain the ability to make phone calls. With the provision of an auditor and the stimulation of the relevant areas in his / her head, speech develops and after the implantation, these children can get rid of their deafness and dumb.

To be able to decide on the implantation of a cochlear implant, the patient must be thoroughly evaluated by a team of experienced specialist ENT physicians, audiologists, psychologists, radiologists, appropriate patient selection and successful outcome. In these evaluations, the evaluation of hearing nerve function, the determination of additional obstacle and need, the investigation of the causes of hearing loss, the general health condition and the state of the ears, the presentation of the internal ear and hearing nerve with CT and MR images taken, assessment of speech and language development and identification of training needs are required.

The operation is planned under general anesthesia by an experienced ENT specialist who is given all the evaluations and decision of the cochlear implant. The operation takes about 2-4 hours. After an attempt made from the back of the ear, the electrode is pulled forward from the opening of the inner ear to the cocoon after the appropriate place is prepared behind the ear to the receiver of the device. The function of the electrodes placed before the operation area is closed is tested. Patients are discharged within a few days. 4-6 weeks after the operation, the speech processor is set up as if it is a normal hearing aid, so that sounds are adjusted audibly so as not to disturb the patient. After that, it is necessary to give special training to the person and to adjust the settings of the device periodically and reset it if necessary. Regular speech and language therapy is especially needed for children. Here the interest and effort of the family is very important. A child of a concerned and diligent family can go to the same schools with their peers who have no hearing problems at the end of the education process.

Cochlear implant implantation which makes children who have congenital hearing loss and who can not hear and speak so they can hear and talk and gain congregation, can hear and talk in a timely manner, and make adult people lose their hearing due to any reason and prevent them from getting out of the society Turgut Ozal Medical Center Ear Nose Throat Diseases Department Dentistry has been in place since 2008. A total of 12 patients, including 9 children with congenital hearing loss and 3 patients with loss of hearing after surgery, were successfully treated by our team in December 2008-December 2010, who were evaluated by the necessary committees as requiring cochlear implantation. These patients had a very positive development in their follow- happily observed.
